To register your child at MCNS you will need to print and complete a registration Package.

• 2010-2011 Session Registration Package, available in Microsoft Word, or Adobe Acrobat PDF

You must also provide your child's immunization record. There are three documents.

• Instructions to follow: Ottawa Public Health Requirements
• What and When: lmmunization Schedules in Ontario
• Form to fill out: City of Ottawa "Child Care Facility - Immunization & Tuberculosis Assessment" form.

Finally the Ministry of Community and Social Services requires that all employees and volunteers, this includes duty parents, provide a criminal reference check before being allowed to participate in the classroom.
